Before you fill this in:

You must be between 25 and 70 years old on the day you drive, not on the day you register.
Your licence must still be valid on the day you drive. We check against the leg date, not against today.
Registering is a claim, not an approval. A committee member checks your licence before you are put on a schedule.
Only club members may be approved to drive rental vehicles. That is a condition of the club's agreement with the rental companies.
